Job Title: Licensed Practical Nurse (LPN)

Summary:
The Licensed Practical Nurse (LPN) provides nursing care and education to patients under the supervision of a Registered Nurse (RN). The Licensed Practical Nurse participates in the planning and coordination of patient care in conjunction with an interprofessional team of medical professionals. The Licensed Practical Nurse reports to the supervisor on duty and executes the nursing plan assigned to each patient.
Responsibilities
The position responsibilities include, but are not limited to:
- Admits, transfers, and discharges residents.
- Administers medications and treatments to patients.
- Monitors patient conditions and records appropriate findings in patient medical records.
- Promotes an environment of compassion by promoting emotional and psychological support to patients, friends, and families.
- Performs wound care.
- Educates patients about treatment plans.
- Documents actions in accordance with facility record keeping standards.
Skills and Experience
- One (1) year of nursing experience (Preferred).
- One (1) year of nursing experience in Geriatric, Hospice & Palliative, and/or Long-term Care experience (Preferred).
- Sympathetic bedside manner.
- Ability to multitask and work interactively within a team.
Requirements
- Graduate of an accredited school of nursing.
- Current active state licensure Licensed Practical Nurse (LPN).
- Current CPR Certification.
